General Description

Nurses coordinate the provision of medical care for patients. Nurses observe, monitor, record and report symptoms or changes in patients’ conditions, and administer medication and treatment to patients.

Minimum Qualifications

Graduate of an accredited school of nursing.

Current New York State Registered Nurse license

Counselor/Therapist

General Description

A counselor/therapist is the primary clinical caregiver for each patient. They assist with the development of clinical treatment tracks that are diversified in language and culture that will meet the treatment and service needs of patients and families.

Role & Responsibilities

Responsible for maintaining and managing patient caseloads.

Responsible for running individual and group counseling sessions (as assigned).

Perform Bio/Psycho/Social assessments on patients  upon intake as needed.

Under the supervision of a master level Qualified Health Provider (QHP), they assist with weekly utilization reviews based on established clinical and administrative protocols.

Asist in the preparation of weekly case conferences and case reviews.

Assist with the clinical discharge process.
Minimum Qualifications

High School Diploma or equivalent with six (6) years of related work experience, or an Associate degree with three (3) years of related work experience

Patient CareCoordinator/Recovery Coach

General Description

The Patient Care Coordinator (“PCC”) acts as the primary hour-by-hour support staff for all patients. The PCC works closely with the nursing and clinical staff, assuring patients the best possible care.

Role & Responsibilities

Regularly communicate with Counselors and Nursing staff regarding patient issues and patient care.

Assure that patients adhere to the program activity schedule.

Encourage positive interactions and conversations amongst patients and staff.

Act as a role model for patients and peers.

Transport patients throughout the facility.

Observe clients for inconsistent behavior and report to appropriate staff (Clinical, Nursing, etc) and include this information in your shift report.

Assist in the de-escalation of behaviorally compromised patients.

Must be willing to conduct intake interviews with newly admitted patients and physically search a patient and their property for contraband.
